The Bureau of EMS has updated the Commonwealth Transfer of Care form template to include an area for ‘next of kin/emergency contact’ information.  Agencies are authorized to use the previous form until supplies are exhausted.  Please contact the Council office for additional information.  The new Transfer of Care form is available on the Bureau of EMS webpage or by clicking here.
